If you are a new patient to Deerfield Dermatology, please print and complete
the Patient Registration Form Please bring this completed document, along with a picture ID and insurance cards to your first appointment in order to expedite the check-in process.
If you are a new patient who will be having a cosmetic consultation or procedure, please also print Skin Care History Questionnaire and Waiver which you can find under Patient Questionnaire, in addition to the Patient Registration.
Returning Patients
Have you moved or changed jobs since your last visit? Has it been three years since you last completed a Patient Information sheet?
The doctors of Deerfield Dermatology require the most up-to-date information on their patients in order to service you better.
